VR Vendor Qualifications Process
Rehabilitation Technology Providers
Step 1: Potential Vendor submits
the following information to the Division of Vocational Rehabilitation,
Vendor Certification Unit:

Standard Vendor Application
-
A copy of Federal Employer’s
Identification Tax Certificate (W-9) or a copy of your Social Security
Card.

Copy of occupational license issued by a
county/city municipality.

Copies of insurance coverage
Step 2: Upon receipt of the
completed application package and supporting documents, the Vendor
Certification Unit forwards the application to the Rehabilitation
Engineering & Technology Program (RETP) at the University of South
Florida
Step 3: The RETP may request
additional documentation/information such as vendor application forms
for
Vehicle Modification, and/or,
Wheelchair/Scooter, and/or,
Driver Rehabilitation Specialist
NOTE: the above form(s) should be
filled only after VR approval of your
Standard Vendor Application as
given in Step 1.
from each interested vendor and/or conduct
an on-site visit in order to make recommendations regarding the vendor
meeting the minimum qualifications. Based on this review, the RETP
submits a written recommendation to the Vendor Certification Unit within
30 calendar days regarding the prospective vendor’s ability to provide
rehabilitation technology products and services.
Step 4: Upon receipt of this
written recommendation, the Vendor Certification Unit will make its
determination and notify the prospective vendor of approval or denial.
